Position Title: Helping Hands Adult Homecare Services Nurse

Position Summary:

The Helping Hands Adult Homecare Nurse provides on-the-job instruction to Homecare Providers relative to specific client needs.
Will make outreach visits to the Helping Hands Adult Homecare clients’ home so that client safety may be assured and the personal care and housekeeping needs of the client are adequately met.
Plans and conducts health and safety trainings to agency employees.

Duties And Responsibilities:

Helping Hands Adult Homecare Services

Participates in annual in-service training for Homecare Providers as directed by the Helping Hands Adult Homecare Supervisor.
Provides assessment, training and supervision to assigned Homecare Providers and clients.
Visits the homes of assigned clients twice yearly and PRN to evaluate the care given to the client and the duties performed by the Homecare Provider.
Completes computerized documentation and assessments in a timely manner.
Maintains regular communication with Scheduler concerning changes and/or concerns regarding a Homecare Provider or a client
Attends Program staff meetings and facilitates discussion on any identified problems as pertinent to the position.
Has the ability to work as part of a team with schedulers, nurses and/or care managers from outside agencies to coordinate client care for the safety of the client (this may also include family members).
Provides backup coverage to Helping Hands Homecare Scheduler and participates in Helping Hands Adult Homecare on-call rotation as assigned by supervisor.
Provides backup coverage to Harbor House Adult Day Services Nurse.

General Duties

Serves as a member of agency committees as assigned.
Will perform evaluation and placement of intradermal TB skin test to employees as required.
Helps to organize and facilitate support groups, including Powerful Tools for Caregivers groups.
Will conduct CPR and First Aid Trainings as needed.
Performs mouth swab drug screens on employees as requested by Human Resources Department.
Assists in locating a contract facility to provide Hep B Vaccines to employees. Will assist with referring employees to the contract facility.
Participate in agency and community trainings in order to retain licensure, promote professional growth, and facilitate problem-solving at both the program and agency level.
Other duties as assigned.

Qualifications:

Demonstrate skills in nursing practice conducive to Helping Hands Adult Homecare Services procedures (observe client in the environment, listen to clients, and patience with the elderly and disabled).
Demonstrate good organizational and communication skills and maintain confidentiality in compliance with HIPAA regulations and federal confidentiality laws.
Ability to utilize infection control procedures.
Must have the ability to lift 25 lbs.
Ability to complete multiple duties and tasks correctly and on time without being prompted.
Willing to learn program specific computer applications.
Ability to work cooperatively with management and staff across all levels.

Minimum Education and Experience:

Possession of a current license to practice as a Registered Nurse (RN) in the State of Michigan.
Minimum one (1) year of Registered Nurse (RN) experience required with homecare experience preferred.

Special Conditions of Employment:

Valid Driver’s License, a working motor vehicle, and proof of current vehicle insurance.
Undergo and successfully pass a State of Michigan LIEN Check and Central Registry Clearance.
Pre-Employment Drug and Alcohol Screen.
Negative TB Test upon hire with follow-up TB tests as required by CFSSWMI policy.
Undergo a background check with no felony convictions and no abuse/neglect convictions.
Must have the ability to work a flexible schedule including some evenings, weekends and holidays, as necessary.
